*As of 3-1-11, Disability Income (DI) and Business Overhead Expense (BOE) Insurance (Forms U4501 NC, U4502 GR, and UC 4403 S) is still issued by The Union Central Life Insurance Company (Union Central) in approved states outside New York, and within New York. Throughout 2011, pending state approvals, Ameritas Life Insurance Corp. will begin issuing DI and BOE insurance in approved states outside New York, and First Ameritas Life Insurance Corp. will begin issuing DI and BOE insurance within New York. Transition dates and new form numbers will be announced upon final approvals by the states.
